    Pharmacological action

    Helogaphen is a tranquilizer of benzodiazepines. This medication has anxiolytic, sedative, hypnotic, anticonvulsant, muscle relaxant action central. The mechanism of action is associated with increased inhibitory effect of GABA in the central nervous system by increasing the sensitivity of the GABA-receptor mediator by stimulating benzodiazepine receptors.

    Pharmacokinetics

    After oral administration Helogaphen is almost completely absorbed from the gastrointestinal tract. After IM administration the absorption is slow. The plasma protein binding is high - about 96%.
    Chlordiazepoxide crosses the blood-brain barrier, excreted in breast milk and crosses the placental barrier.
    T1/2 of this drug is variable and is 5-30 hours, T1/2 of the main active metabolite (desmethyldiazepam) is a few days. Helogaphen is excreted in the urine in unchanged form and as conjugated metabolites.

    Why is Helogaphen prescribed?

    Neuroses, insomnia, nervous tension, anxiety, irritability, reactive depression, withdrawal syndrome, convulsive state, or threat thereof, of NDC, menopausal syndrome, premenstrual tension, in preparation for operations.

    Dosage and administration

    For oral administration of Helogaphen the dose for adults is 5-10 mg 2-4 times / day. In states of stress and anxiety, combined with sleep disorders in 10-20 mg 1 time for 1-2 hours before bedtime. In psychiatric practice the initial dose is 50-100 mg / day and then gradually increase the dose up to 300 mg / day. The multiplicity of applications is 3-4 times / day.     Helogaphen side effects, adverse reactions

    CNS and peripheral nervous system: possible drowsiness, dizziness, muscle weakness; rarely - confusion, depression, blurred vision, diplopia, dysarthria, headache, tremor, ataxia; in rare cases - paradoxical reactions (agitation, anxiety, sleep disturbances, hallucinations). After IV administration sometimes observed hiccups. With prolonged use may develop drug dependence, memory impairment.
    Digestive system: rarely - constipation, nausea, dry mouth, excessive salivation, in single cases - raising the level of transaminases and alkaline phosphatase in the blood plasma, jaundice.
    Endocrine system: rarely - increased or decreased libido.
    Urinary system: rarely - urinary incontinence.
    Cardiovascular system: when chlordiazepoxide administered IV it may be slight reduction in blood pressure.

    Helogaphen contraindications

    Myasthenia gravis, severe respiratory insufficiency, severe chronic hypercapnia, phobias, obsessive-compulsive disorder, chronic psychosis, indicate a history of alcohol or drug dependence (except for acute withdrawal), increased sensitivity to chlordiazepoxide and other benzodiazepines.



    Using during pregnancy and breastfeeding

    You should not use Helogaphen in the I trimester of pregnancy, except in cases of extreme necessity. It should be noted that the application of chlordiazepoxide during pregnancy may significantly change the fetal heart rate. When used in obstetrics at the doses recommended in order to facilitate childbirth, newborns, more frequently in premature infants, it may be temporary muscular hypotonia, hypothermia, respiratory failure.     Special instructions

    Helogaphen should be removed during the development of paradoxical reactions (severe agitation, anxiety, sleep disorders and hallucinations).
    Avoid IA administration.
    In the period of treatment avoid alcohol.

    Helogaphen drug interactions

    When this drug applied simultaneously with:
    - antipsychotic drugs (neuroleptics), antidepressants, sedatives, hypnotics, anesthetics, analgesics, ethanol enhanced the inhibitory effect on central nervous system, especially when administered parenterally.
    - concomitant use of antacids can delay but not reduce the absorption of chlordiazepoxide.
    - hormonal contraceptives increases the effectiveness of chlordiazepoxide.
    - with muscle relaxants may enhance the action of muscle relaxants, with levodopa - may suppress antiparkinsonian action.
    - theophylline at low doses perverts sedative effect of Helogaphen.
    - phenobarbital and phenytoin may accelerate the metabolism of chlordiazepoxide. In rare cases this medication inhibits the metabolism and enhances the action of phenytoin.
    - may increase the toxicity of cyclophosphamide.
    - cimetidine, omeprazole, disulfiram may increase the intensity and duration of action of this medicine.

    Helogaphen in case of emergency / overdose

    Symptoms: CNS depression of varying severity (from lethargy to coma) - marked drowsiness, lethargy, weakness, decreased muscle tone, prolonged confusion, disorientation, slurred speech, depression of reflexes, coma; also possible respiratory depression, hypotension.
    Treatment: induction of vomiting, gastric lavage immediately, monitoring vital signs, symptomatic therapy, IV infusion of solutions (to increase urine output), providing adequate ventilation; in case of hypotension - the introduction of norepinephrine. With the development of excitation it should not be used barbiturates. As a specific antidote used benzodiazepine receptor antagonist flumazenil (in hospital). Hemodialysis is ineffective.
