    Pharmacological action

    Tegod is a medication regulating the motor function of the gastrointestinal tract, a partial agonist at serotonin 5-NT4-receptors. This drug has a selective effect on the gastrointestinal tract. It stimulates serotonin 5-NT4-sensitive receptors in the terminals (afferent) neurons, and gastrointestinal tract causes the release of a number of mediators (eg, peptide, calcitonin gene-related). Tegaserod enhances peristalsis, increases the basal motor activity and normalizes impaired propulsive activity throughout the gastrointestinal tract. By improving the education of cyclic AMP under the influence of tegaserod it is increased secretion of chloride ions and water crypts of the intestine. Under the influence of this medication it is reduced the number of pulses generated by afferent neurons in response to stretching of the rectum.
    Tegod has a high affinity for serotonin 5-NT4-receptors of man, it does not have any significant affinity for 5-HT3 and dopamine receptors.
    In patients with irritable bowel syndrome with constipation predominance of tegaserod accelerates the passage of intestinal contents on the small and large intestine. This drug promotes normalization of stool consistency and increases the number of bowel movements, reduces the severity of symptoms (discomfort / abdominal pain, constipation), characteristic of irritable bowel syndrome.

    Pharmacokinetics

    After oral administration Tegod is rapidly absorbed from the gastrointestinal tract. Time to Cmax is approximately 1 hour. In the fasting bioavailability is approximately 10%. Food reduces the bioavailability of this medication by 40-65% and Cmax about 20-40%.
    Plasma protein binding is 98%. Penetrates through the BBB to a small extent.
    Tegod metabolized primarily by first-pass hydrolysis in the stomach with an acid and catalase by direct glucuronidation.
    In in vitro studies it have been shown that this medicine does not inhibit the isoenzymes CYP2C8, CYP2C9, CYP2C19, CYP2E1 and CYP3A4. At the same time it can not been exclude the inhibition of isozymes CYP1A2 and CYP2D6. In the study of metabolism in humans it was been showed that tegaserod does not suppress the activity of any of the above cytochrome P450 isoenzymes.
    Nearly two thirds of an oral dose is excreted unchanged in feces; the remainder of the dose is excreted in the urine, mostly as the main metabolite.
    Significant accumulation in plasma of this drug is not observed.
    In people with mild or moderate hepatic impairment it has been shown an increase, compared with healthy individuals, the mean AUC or Cmax of 43% and 18%.

    Tegod is taken orally before eating. The recommended dose is to 6 mg, 2 times / day.

    Tegod side effects, adverse reactions

    May include diarrhea, abdominal pain, nausea, flatulence, headache, dizziness, back pain, flu-like symptoms.
    In a placebo-controlled studies on safety found that patients treated with tegaserod, increased risk of angina attack, stroke, increasing chest pain, which can lead to angina attack.

    Severe hepatic failure, children's age, pregnancy, lactation, hypersensitivity to tegaserod.
